Scott, do you mean that islets were taken from a living twin for transplant into the diabetic sibling? If this is the case, then the islets could have died for a million reasons other than autoimmunity. To begin with, one can only remove a very small piece of pancreas, and the number of islets that survived after isolation would be minimal. I am not surprised that the transplant failed, but what evidence is there that autoimmunity was the cause?
