The latest Lancet has an overview of the three major technologies being applied in the development of a continuous glucose monitor.
One interesting aspect of the interstitial fluid monitoring method (such as the Cygnus GlucoWatch) is the delay between blood glucose changes and interstitial glucose changes. I always believed the effect of this delay would an increased risk of failing to detect a low blood glucose state, as the institial fluid would not have caught up to the blood in a period of rapidly falling glucose. According to the article, the opposite is true. Apparently, glucose in interstitial fluid falls faster than in blood, providing an early warning of impending lows.
